Branch: Metairie, LA | Schedule: M-F, 8-5 | Coverage: West bank South of the river Waggaman, Westwego, Marrero, Gretna, Belle Chasse, etc. all the way south

Ensure all activities align with the vision of Elara Caring leadership and Home Health team standardsProvide skilled nursing assessments and interventions based on patient/family needs, as documented in clinical records and interdisciplinary reportsManage a caseload by assessing, planning, coordinating, implementing, and evaluating individualized plans of careConduct initial evaluations and ongoing re-evaluations to identify physical, psychosocial, and environmental needsDeliver follow-up visits as appropriate to patient, family, or facility requirementsCommunicate significant findings, changes in condition, and safety concerns to the Clinical Supervisor, physician, facility staff, and care teamRevise care plans in response to patient needs and collaborate with the interdisciplinary team for continuity of careProtect patient and staff privacy, ensuring compliance with HIPAA standardsWhat You’ll Need
Current, unrestricted RN license (state-specific)Minimum of 1 year of nursing experience in a clinical care settingStrong clinical assessment and critical thinking skillsAbility and willingness to travel within branch/office coverage area (up to 50%)Reliable vehicle, valid driver’s license, and current auto insurance (per state requirements)Associate’s or Bachelor’s Degree in Nursing (preferred)Prior Home Health or Hospice experience (preferred)Experience with HomeCare HomeBase (preferred)As a growing organization, we invite you to share your information with us for consideration for future career opportunities. Reporting Structure
This position reports directly to the Clinical Manager or Clinical Supervisor.
