Associates are eligible to apply for a transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Probationary period is four (4) months for full-time employees, and six (6) months for part-time employees.

As a Home Health Registered Nurse you will:

Act as liaison between patient, family, and physicianProvide updates on the patient condition to family and physicianEnsure all in-home medical equipment are in working orderDocument all details of patient’s condition, progression and regression activityOversee required physical therapy or rehabilitation exercisesContact emergency services when patients need to be taken to the hospitalPossess knowledge of Home Care reimbursement processPossess knowledge of OASISHave reliable transportation as you go to/from home visitsCan walk up/down six (6) flights of stairs as needed

 

Requirements:

Minimum of one (1) year hospital Med Surg experience2 years of Home Health experience | Certified Home Care (CHHA) experience (Preferred)NYS RN License & Registration in good standingBachelor's degreeInfection Control certificateCurrent CPR certificationValid Driver License, Registration and Automobile InsuranceKnowledge of a second language a plus
