This is a Full-Time position working daylight from 8 AM- 4:30 PM. On-call shifts and rotating weekends are required.

Responsibilities:
Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to the patient. Completes an initial and comprehensive assessment of the patient. Initiates, coordinates, and ensures ongoing communication with the physician and other members of the interdisciplinary health care team, including evaluating patients and family-centered health teaching. Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care. Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members by the physician’s orders and patient needs. Assigns other interdisciplinary team members as appropriate to assist in the delivery of services as ordered on the plan of care to the patient. Completes all necessary documentation, which may include visit assessment, plan of treatment, and verbal orders, accurately and promptly in the electronic documentation system while in the patient’s home and within the agencies and regulatory standards. Data syncs (transmits) information same day. Participates in weekend and on-call coverage rotation for the agency as per BU guidelines. Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care.
