Responsibilities:

Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient’s condition to the appropriate professional staff. Attends compliance training and adheres to the organization’s standards of conduct, policies, and procedures. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional. Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. Proven ability to facilitate change. Maintains and completes documentation accurately and promptly while in the patient’s home and within the agency and regulatory standards. Participates in all required agency meetings.
