**Responsibilities:**
+ Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff.
+ Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
+ Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
+ Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional.
+ Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
+ Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
+ Proven ability to facilitate change.
+ Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
+ Participates in all required agency meetings.
+ Must maintain annual competency evaluation scores as determined by regulatory requirements

+ High School Diploma or GED - Required
+ One (1) year of experience - Preferred
+ Certified Nurse Aide - Preferred
+ Minimum of 75 hours of Nurse Aide training - Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire. **Licensure, Certifications, and Clearances:**
+ Automotive Insurance
+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
+ Driver's License
+ Act 33
+ Act 34
+ OAPSA
