Home Care Aide Erie County
UPMC Home Healthcare and Family Hospice is hiring a Home Care Aide to support Erie County. This is a Monday through Friday, 8:00 AM-4:30 PM position with rotating weekend shifts, providing exceptional care to patients in their place of residence. This is a field position, making home visits to both home health and hospice patients. Responsibilities: + Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff. + Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. + Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. + Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals. + Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. + Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. + Proven ability to facilitate change. + Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. + Participates in all required agency meetings. + Must maintain annual competency evaluation scores as determined by regulatory requirements + Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. + Proven ability to facilitate change. + Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. + Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. + Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. + Participates in all required agency meetings. + Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in the patient's condition to the appropriate professional staff. + Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. + Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals. + High School Diploma or GED - Required + One (1) year of experience - Preferred + Certified Nurse Aide - Preferred + Minimum of 75 hours of Nurse Aide training - Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire. **Licensure, Certifications, and Clearances:** + CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire + Automotive Insurance + Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) + Driver's License **UPMC is an Equal Opportunity Employer/Disability/Veteran**
