We are seeking to fill the HOME-ARP Case Manager position and add valuable members to our team to accomplish our mission!
Veterans Inc. is seeking a HOME-ARP Case Manager to join the Veterans Inc. Outreach Team. This position plays a key role in engaging individuals and families experiencing or at risk of homelessness, including veterans and non-veterans, by delivering field-based case management services via the Mobile Outreach Center.
WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
+ Mobile Outreach & Engagement: Operate and maintain the Mobile Outreach Center (RV), including driving to scheduled service locations across Worcester. Engage directly with clients in the field, providing essential needs items (e.g., food, clothing, hygiene kits) and information about services.
+ Case Management Support: Assist individuals with completing intakes, eligibility screenings, and assessments. Develop Individual Service Plans (ISP) that reflect client goals, including housing, health, recovery, and self-sufficiency. Provide ongoing monitoring and follow-up.
+ Support to Recovery Coach & Outreach Team: Act as an assistant to the Certified Recovery Coach and EOVS-Outreach Specialist by supporting outreach logistics, follow-ups, and client navigation. Participate in recovery and re-entry plans for individuals in treatment or post-treatment.
+ Resource Coordination: Facilitate internal and external referrals to housing, medical, psychiatric, employment, legal, and financial resources. Leverage Veterans Inc.'s partnerships with community providers to connect clients to needed services.
+ Documentation & Compliance: Maintain timely, accurate, and audit-ready documentation in HMIS and organizational systems. Ensure proper intake forms, service notes, and client records are up to date.
+ Client Advocacy: Provide culturally competent, trauma-informed support while maintaining professional boundaries. Advocate for client access to care and services, especially for hard-to-reach or high-barrier populations.
+ Vehicle Care & Safety: Ensure the Mobile Outreach Center Unit is maintained, fueled, and road-ready at all times. Perform basic cleaning, inventory checks, and report any maintenance requirements (regular or emergent).
WHAT YOU MUST HAVE:
+ Bachelor's degree in Human Services, Social Work or other related field required. An Associate's degree or military service with significant field experience will be considered.
+ Minimum of 2 years' experience in outreach, case management, recovery support, or homeless services.
+ Demonstrated ability to work independently in a mobile or community-based setting.
+ Experience working with vulnerable populations, including individuals with substance use and behavioral health disorders.
+ Frequent travel is required. Must have a valid driver's license and own personal, reliable transportation.
+ Must be able to successfully pass a CORI/Background records check.
+ Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
