JOB SUMMARY:

We are seeking a compassionate and dedicated Holistic Community-Based Nurse with training in Lifestyle Medicine to join our Community Health Outreach team. This role involves promoting wellness, preventing chronic disease, and improving health outcomes by integrating evidence-based lifestyle interventions with community-based nursing practices. The ideal candidate is passionate about culturally competent care and empowering individuals and families to make sustainable health behavior changes.

Qualifications

Current licensure by the New Jersey Board of Nursing as a Registered Professional NurseCurrent and valid driver s license, registration, inspection sticker, and appropriate level of insurance coverage required, to include collision to perform the essential duties of the positionCertification or formal training in Lifestyle Medicine, e,g, American College of Lifestyle MedicineMinimum 2 years of experience in community health, public health nursing, or related fieldStrong communication, education, and motivational interviewing skillsExperience working with diverse, underserved, and vulnerable populationsAbility to work independently and as part of a team in a dynamic community settingBilingual skills are a plus

Education:

Graduate from an accredited School of NursingBSN preferred

Conduct comprehensive community health assessments to identify population health needsDesign and implement lifestyle medicine interventions focused on nutrition, physical activity, sleep, stress management, substance use, and social connection,Collaborate with interdisciplinary teams to deliver coordinated, patient-centered care,Provide individual and group education sessions in community settings, e,g, schools, faith-based organizations, shelters,Develop culturally tailored outreach strategies that address social determinants of health,Maintain accurate documentation and collect data to evaluate program outcomes,Serve as a liaison between community members and local health resources and services,Advocate for policies and programs that support holistic health and community well-being,
