Are you living your life's work? **Job Title:** Hepatitis C Care Navigator **Department:** PMC Infectious Disease Maps **Location:** Penn Presbyterian Medical Center - 51 N 39th St **Hours:** Mon-Fri office hours **Summary:** + The Care Navigator reports to the ID Program Manager. The Navigator is responsible for assuring that patients identified through the inpatient Hepatitis C screening program are linked and engaged in quality medical services that result in treatment appropriate for their diagnosis. The Care Navigator will work directly with patients, providers, and key stakeholders across the health system and in the community to achieve this goal. The navigator will follow patients to ensure successful linkage is attained and treatment is achieved. Position may have a focus on Hep C or HIV per operational need. **Responsibilities:** + Conduct an accurate patient-centered, cultural, sex, and gender-sensitive risk assessment with clients and thoroughly explain the importance of engagement in care and achieving cure for infectious diseases. + Provide emotional support, counseling, and advocacy for patients throughout the process of treatment for infectious diseases. + Facilitate and ensure that all patients receive the standard of care associated with infectious diseases, regardless of the location of their treatment. + Assist patients, families, other caregivers, and facilities in coordinating the treatment of infectious diseases for patients. + Maintain relationships with facilities who partner with Infectious Diseases. + Work on a multi-disciplinary team of physicians, nurses, pharmacists, social workers, and other members of the healthcare system. + Maintain documentation in the Electronic Medical Record as well as spreadsheets used to coordinate care of patients and track outcomes. + Assist with benefits counseling including assessing for state coverage, patient assistance programs, copay assistance, and navigating insurance barriers. + Performs duties in accordance with Penn Medicine and entity values, policies, and procedures + Other duties as assigned to support the unit, department, entity, and health system organization **Education or Equivalent Experience:** + Bachelor of Arts or Science (Required) + Education Specialization: Social Work, Public Health or related field. + 2+ years related experience (Required) We believe that the best care for our patients starts with the best care for our employees.
