**Healthcare Occupational Case Manager**
We're looking for a skilled **Healthcare Occupational Case Mgr** to join our team.
The primary purpose of the position is to direct and coordinate the delivery of timely, appropriate, and cost-effective healthcare through the case management process. This includes managing disability, urgent and emergency care, immunizations, and work limitations, as well as developing transitional return-to-work plans for workers' compensation and non-workers' compensation injuries and illnesses. The role is also responsible for after-hours coordination of urgent and emergency care services and reports to the OHS Lead Clinician.
**Pay:** $40/HR (21.66/HR)
**Shift:** HYBRID - M-F: 6:45 AM - 3:30 PM
_*_ **_Important Schedule Note:_** _The position will have an after-hours on-call rotation that runs weekly and rotates among staff after training is complete._
**Location:** HYBRID - Chicago, IL
**Primary Duties and Responsibilities:**
+ Identifying cases with high-frequency and high-risk injuries/illnesses, and performing ongoing evaluation and treatment plans.
+ Determining essential job functions and implementing necessary accommodations, limitations, and part-time work to reduce the length of disability in high-risk cases.
+ Identifying cases requiring physician-level review for case management and coordinating the review proce
**Qualifications:**
+ 1-5 years of Nursing experience.
+ Education Level/Type: Registered Nurse, Nurse Practitioner, or Physician Assistant (licensed in the state where service is provided).
+ Basic computer knowledge and cell phone skills.
+ Knowledge and understanding of case management for workers' compensation, disability, limited duty, safety, Industrial Hygiene, healthcare plans, regulatory medical requirements, and external healthcare resources.
+ Ability to work independently and as part of a team.
+ Preferred experience in Nursing Case Management or Occupational Health.

**Pay Details:** $40.00 per hour
