Key Responsibilities:
Meet with clients to encourage service utilization and monitor their needs.
Develop and maintain partnerships with community organizations to assist clients in accessing insurance, medical care, transportation, and other resources.
Provide case management services to support clients' individualized needs.
Assist clients with Medicaid enrollment or securing an appropriate insurance plan.
Coordinate and track referrals to primary and specialty care providers.
Re-engage clients who have not maintained medical treatment.
Model professional behavior consistent with agency expectations and program philosophy.
Perform other duties as assigned by the Clinical Supervisor, Program Director, Vice President, or Executive Team member.
Education and/or Experience Qualifications:
Bachelor’s Degree in social work, counseling, psychology, psychiatric nursing, or a closely related field;
OR
Four (4) years of relevant, qualifying full-time experience in human service delivery, demonstrating skills in developing productive client relationships and navigating complex service systems.
OR
A combination of education and relevant, qualifying experience (substitution allowed: one year of experience per year of education).
Additional Qualifications:
Knowledge of case management and rehabilitation methods as they relate to mental health and/or substance use disorders.
Familiarity with community resources for mental health and substance use.
Basic computer skills, including proficiency with Microsoft Word and Excel.
Strong communication skills for verbal and written reports.
