Duties Description The functional title for the candidate selected for this position is Placement Coordinator.Placement Coordination• Under the supervision and direction of the Treatment Team Leader (TTL), the staff will receive and prioritize screening packets for available vacancies and will coordinate the process of pre-screening new intakes, review and analyze case files, and collaborate with Team Leaders as well as consult with key personnel to facilitate screenings. The placement coordinator will also embark on visits to group homes, hospitals, rehabilitation centers, and other facilities as applicable. • Cultivate relationships across diverse communities to provide new opportunities for Individuals with Intellectual Disabilities (IDD) amidst housing in State Operations.• The Placement coordinator will initiate referrals to clinics based on needs of individual to secure stable residential placement. All services are connected to the individual’s level of functioning. • Interact with regional office, care management, families, hospitals, Mental Hygiene legal services, vocational providers to obtain needed documentation with which subsequent services are coordinated.• Chair of the regular placement meetings (remote and in-person) including screenings designed to foster relationship building, collaboration, and promote equitable services for IDD population. This includes strengthen relationships with TTL, and QIDPs, identifying and addressing improvements to working together to authentically support IDD population including implementation of equitable and best practices and continued communication strategies.• Assist in conducting outreach, recruitment, and screenings for Individuals. Maintain State Operations Vacancy Tracking on a weekly basis; Provide individual workshops to Self-Advocates and other Stakeholders. Canvas local Regional Service Field Office to solicit housing opportunities, etc.• Track and maintain a list of all referrals from Regional Service Field Office, Division of Statewide Services, and Director to Director. Submit screening forms and other data driven documentation to appropriate parties regarding potential placement opportunities. Complete/update necessary documentation inclusive of access to Choices and TABS.• Attend Level of Supervision (LOS), Comprehensive Adult Transitional Housing (CATH) and other meetings as required.• Prepare Vacancy reports noting the vacancies that exist in the district and ensuring its distribution to central office and local staff so appropriate referrals to fill vacancies can be provided.• Prepare residential profile reports for distribution to ensure that individuals matching the profile of the residence will be referred to maximize the opportunity for a smooth transition for the individual. QIDP/Other• Carry a small caseload of residential homes to provide coordination of residential habilitation services. Coordinate holistic, person centered approach when completing tasks such as coordination of services, creation of accurate Staff Action plans, review and approve Life Plans, keep schedule for meetings, generate Safeguard Summaries, facilitate staff training on documents and necessary supports, and be cognizant to recognize when treatment plans for the individuals supported need reviews, revisions and changes when applicable.• Ensure all documents and efforts follow OPWDD and Medicaid/Medicare regulations and practice requirements.• Liaise with other QIDP to ensure continuity of supports and services in line with Brooklyn DDSO policies and procedures.LinkedIn: https://www.linkedin.com/company/nys-office-for-people-with-developmental-disabilities/posts/?feedView=all#LI-JJ1
Minimum Qualifications Currently serving as a permanent Habilitation Specialist 1.ORMust have one year of service in a title eligible for a 70.1 transfer in accordance with Civil Service law.OROpen Competitive Qualifications:A Bachelor’s or higher-level degree in a human services field**For the purpose of these qualifications, a degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology and therapeutic recreation (including concentrations in art, creative art, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.
Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.• Please note that starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for Grade 14 ($53,764) and Downstate Adjustment ($4,000). Downstate Adjustment is added to the job rate ($68,630).• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.• Preference will be given to OPWDD employees impacted by closures.
