Group Visits Champion, Primary Care **Requisition Number:** 9851 **Location:** CHA One Cabot Care Center **Work Days:** 40 **Category:** Professional and Management **Department:** Weight Management and Wellness **Job Type:** Full time **Work Shift:** Day **Hours/Week:** 40.00 **Union Name:** SH Laborers 381 Summary: The Group Visits Champion for the comprehensive Weight Management and Wellness (WMW) program plays a pivotal role providing patient navigation and supporting Groups across sites and departments for Cambridge Health Alliance. Responsibilities: Works with providers and specialists to set the schedule for WMW group visits, and support groups across the comprehensive WMW program (Primary Care, Endocrine, Nutrition, Behavioral Health, Clinical Pharmacy, and Bariatric Surgery). Coordinates and manages Group calendar and invites. Staffs a work queue of referrals to the group and coordinates outreach to eligible patients. Directly schedules patients for group visits Participates in patient recruitment, provides psychoeducation and coaching to support patient motivation and attendance. Provides patients with instructions and support for technology set up. Provides patient reminder calls. Sends pre-visit questionnaire through MyChart and collects responses, as assigned. Huddles with the clinical team prior to and after the group. Recognizes patient related issues and passes on accurate information to appropriate personnel. Calls patients who are not present at the start of the group to troubleshoot any technology issues. Provides leadership and coordination support during group visits, as needed and as available. Tracks show rates and clinical outcomes for groups as assigned. Collects feedback from patients and compiles this for qualitative data of patient experiences, as assigned. Participates in identified meetings to discuss challenges, propose improvements, and share best practices with Primary Care and other CHA stakeholders involved in group visits. Participates in continuing education sessions as needed. Provides training support to other group champions in the group champion role. Administratively supports the Program leadership team, including but not limited to the Program Director, PA Navigator(s), and Medical and Surgical Directors. Requirements: Education/Training: Minimum of high school diploma or equivalent Linguistic capabilities: Fluency in English required. Fluency in Spanish, Portuguese or Haitian Creole desirable. Ability to work through a CHA interpreter when needed. Work experience and skills: Minimum of two years’ administrative duties in a clinical setting preferred. Experience facilitating behavior change in a wellness, weight management, nutrition, or bariatric surgery setting highly preferred. Tech-savvy with ability to coach patients and staff explaining and practicing how to use technology, especially video conference software and My Chart portal. Demonstrated experience with scheduling and related coordination and organizational skills. Clear communicator, including comfort working with patients with substance use disorders and other mental health conditions. Collaborative team member coordinating with patients and clinical team. Ability to prioritize and multi-task responsibilities. Proactive - anticipates needs and challenges and works to resolve. Ability to interpret general and technical data in relationship to specific job responsibilities. Certifications such as Certified Health Coach or Certified Weight Management Specialist are desirable, but not required.
