Baylor Scott & White Health is seeking a dedicated, Board-Certified (or Board-Eligible) Internal Medicine Physician with a strong interest or fellowship training in Geriatrics to join our Baylor Scott & White Plano, Texas clinic. This is an exciting opportunity for a compassionate physician to provide specialized, high-quality care to older adult patients within an integrated healthcare system that values excellence, innovation, and a team-based approach.

This is a specialized clinic providing system-aligned care for high-risk Medicare Advantage and Medicare members.  The goal of the clinic is to add value to the overall health of the patient.  The physician will serve a key role in patient’s disease management, health improvement and satisfaction, while decreasing unnecessary utilization and total patient costs.

As a Geriatric-focused Internal Medicine Physician, you will lead efforts in delivering age-sensitive, comprehensive care to our growing senior population. You will manage complex medical conditions, emphasize preventive care, and coordinate services to address the unique needs of older adults. Key Responsibilities

\n\nProvide comprehensive geriatric assessments and primary care for high-risk adult and elderly patients.\nManage chronic diseases common in older adults (e.g., dementia, heart failure, osteoporosis, diabetes, polypharmacy).\nCollaborate with a multidisciplinary care team including physician assistant/nurse practitioners, social workers, pharmacists, physical therapists, and specialists in palliative and behavioral health.\nAbility to build relationships with practicing physicians and offer consultation on patient eligibility to join geriatric panel.\nPromote functional independence and quality of life through individualized care planning.\nParticipate in quality improvement and utilization initiatives specific to geriatric populations.\nMaintain accurate and timely documentation in compliance with organizational protocols.\n\n\n

Qualifications

\n\nDoctorate in Medicine (MD or DO)\nLicensed to practice in the state of Texas by the Texas Medical Board\nBoard-certified or actively pursuing board certification in Internal Medicine (American Board of Medical Specialties or Bureau of Osteopathic Specialists)\nGeriatric fellowship training or extensive experience in the care of older adults is preferred\n
