Full-Time On-Call Hospice Visit Nurse-Homecare patients-Columbia and Greene Counties
The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in the homecare setting after hours on call.
Schedule is Monday to Friday 4pm-12am with an every other weekend rotation.

As an RN you will manage patients in their home and respond to symptom management needs and coordinate with our team to provide end of life support to patients and families. This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end-of-life care, social workers, chaplains, complementary therapists and volunteers).What you will need:
NYS RN Licensure.Associate's Degree in Nursing, BSN preferred.1-2 years RN experience required-preferably in homecare or acute care.Previous hospice experience preferred but NOT required.The RN must be able to communicate effectively, verbally and written.Valid Driver's License and Reliable vehicle.High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.Community Hospice provides a thorough orientation, preceptors and a supportive work environment.

Pay Range: $35.00-$47.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
