Job Summary: 
Join our compassionate and collaborative team as a Family Support Provider, where you will make a meaningful difference in the lives of families with children facing mental health challenges. In this role, you will empower parents and caregivers by providing essential support and resources, helping them navigate their child's emotional needs. You will play a vital role in developing wellness plans and coaching families towards improved health outcomes. As a Family Support Provider, you will work closely with Community Support Specialists to create a supportive network for families, guiding them through the complexities of mental health services. Your efforts will be instrumental in facilitating essential services for children and families in need.

Key Responsibilities: 
• Assist and coach families to enhance their understanding of their child's needs, fostering positive relationships with child and family team members. 
• Provide valuable resources such as videos, books, and articles, while offering moral support during crises. 
• Help prepare for meetings and engage in activities defined by the treatment team and family. 
• Develop individualized service plans tailored to the family's needs and participate in staffing for continuity of care. 
• Facilitate outside referrals for issues not addressed within the treatment milieu. 
• Assist with intake processes and represent the agency professionally. 
• Document all services provided in accordance with standards. 
• Provide crisis intervention when necessary and facilitate group education sessions. 
• Obtain training for professional development and participate in activities that promote recovery and resilience. 
• Assist parents/caregivers in coping with a child with serious emotional disturbances, modeling resilience through peer support. 
• Offer individual or group services focused on education and support, sharing lived experiences of recovery. 
• Help parents/caregivers recognize their resilience, connect with community resources, and develop a support network. 
• Encourage independent choices and proactive involvement in their child's treatment, identifying strengths and resources for recovery. 
• Adhere to system policies and the Code of Conduct. 
• Demonstrate understanding of health service systems and assist program participants in securing resources for healthy living. 
• Conduct assessments to aid in developing individualized family plans. 
• Engage families in the wellness process, serving as advocates in agency and community settings. 
• Monitor progress and provide support and coaching, ensuring the implementation of family plans. 
• Identify options for agencies to meet family needs and build positive relationships with family members. 
• Facilitate presentations on local resources and services, assisting families in learning health management skills and self-advocacy. 
• Collaborate with team members to connect with community resources and plan wellness activities. 
• Participate in organizational processes as directed by leadership. 
• Serve as a member of a multidisciplinary team, supporting the development of family plans based on individual strengths and needs. 
• Perform all other duties and responsibilities as assigned. 

 

 

Education, Experience, and/or Credential Qualifications: 
• A High School Diploma or GED is required. 
• Must be at least 21 years of age. 
• Must meet the state qualifications required to obtain the Family Support Provider credential.
