Salary: $38 / Hour Family Care Service Coordinator $38/hr - (Includes Weekly Pay & Full Benefits If Needed) Monday to Friday - 9am to 5pm This role is to oversee home liaison's that act as a bridge between patients that age out of school programs & are placed in foster homes. **MUST HAVE SUPERVISORY EXPERIENCE** (will be overseeing home liaison's) Principle Responsibilities: • Advocate for each enrolled individual who requires or chooses to have an advocate • Ensures timely completion of all necessary waiver enrollment processes and annual redetermination of the need for level of care in an ICF/MR. • Listens to the individual and advocate to understand what is important to the individual. • Assists with planning and deciding which services or supports are priority. • Design and completes (within 60 days of the individual moving into Family Care) the individual’s approved ISP and gives copy to the individual, advocate, family care provider, and other service providers.
