Summary The Facility Program Manager (FPM) functions at the highest level of clinical practice, works independently under their scope of practice to directly care for patients. This position will function as a member of the medication safety team and the infectious diseases team and will serve as a mid-level provider to initiate, modify, or discontinue medication therapy and as a consultant for medication therapy management services under the confines of their scope of practice. Reviewing and assessing compliance with VHA Directives/Handbooks, USP, FDA, DEA, AWE, OIG and Joint Commission standards and assisting with the development and implementation of policies and procedures to ensure compliance. Working with the National Center for Patient Safety, the Center for Medication Safety, the Institute for Safe Medication Practices, and FDA, disseminates medication related patient safety issues to appropriate clinical staff and committees and collects and analyzes JDD VAMC data as necessary. Working closely with pharmacy management, patient safety, and clinical leadership to resolve medication safety issues and ensures that processes for prescribing, dispensing, and monitoring medication therapy minimize the potential for adverse medication events. Reviews and evaluates medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes. Orders, reviews and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy. Participates in care coordination by designing pharmaceutical care plans if needed and documents interventions performed in electronic health record, allowing for seamless transitions of care and effective communication within the patient's healthcare team. Utilizes motivational interview skills with patient, family, caregiver and other healthcare professionals to collect patient-specific data, conduct medication history taking to identify problems and individualize care to complement patient's medication therapy history, formulate a pharmaceutical care plan, and identify additional data needed for medication review assessment and medication reconciliation as part of component provider duties. Collaborates with providers to develop, implement, and maintain systems and tools that promote optimal antimicrobial use and desirable clinical outcomes through use of safe, cost-effective drug therapies. Provides academic detailing and education on antimicrobial stewardship and infectious diseases topics to other healthcare professionals including providers, pharmacists and nurses according to local policies. Serves as an authoritative information source on antimicrobials and their uses. Provides both written and oral drug information to physicians, nurses and other healthcare professionals as well as to patients and their caregivers. Develops and updates policies and procedures defining operational requirements of Medication Safety and Infectious disease Programs. Manages the sharing of data and information for Patient Safety and Pharmacy utilizing tools such as Excel, dashboards and SharePoint sites. Actively serves and participates in NWI, VISN and National committees and workgroups. Responsible for ambulatory care, acute care, or CLC pharmacy activities involving highly innovative methods of health care delivery. Provide medication reconciliation and patient medication counseling verifying and documenting patient understanding of the medication, proper medication use, expected outcomes, how to obtain refills of medication and how the current prescription will be processed. Maintains workflow patterns that assure timely delivery of outpatient ambulatory care pharmacy services, ensures accurate dispensing and record keeping documentation, and oversees technical and nonprofessional staff in the outpatient pharmacy setting. Participates in quality improvement activities, pharmacoeconomic initiatives, committees, and workgroups in support of the pharmacy service strategic plan in all areas of the outpatient pharmacy section. Review pharmacy orders to include patient information, dose, route, frequency of administration, patient allergies, drug interactions, incompatibilities, duplication, and priority. Finish, input and/or verify information and orders in the computer system if assigned; and dispense medication in accordance with dosage form requirements (outpatient, unit dose, and IV admixture). Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Creditable Experience Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for- year basis. The pharmacy residency program must be accredited by the American Society of Health- System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Grade Determinations: GS-13 Facility Program Manager: (a) Experience: In addition to the basic requirements, candidates must have 1 year of experience equivalent to the next lower grade level. Examples of GS-12 level equivalent experience include but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): a) Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b) Expert understanding of regulatory and quality standards for their program area. c) Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d) Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e) Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. The full performance level of this vacancy is GS-13. Physical Requirements: This position may require light to moderate lifting and carrying; up to 44 pounds. Requires routine standing, bending and stooping/squatting. As needed - pushing, walking, standing, reaching over shoulder, use of fingers, both hands and legs required, walking and standing up to two hours per tour, near vision correctable at 13' to 16' to Jaeer 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, hearing (aid permitted).
