Exercise Physiologist - Norwalk Hospital Cardiac Rehab - Norwalk, CT
*Description* *Title:* Exercise Physiologist - Norwalk Hospital Cardiac Rehab, Norwalk, CT. Full Time, 36 hours per week, 8 hour shifts Monday - Friday, 8:00am - 4:30pm. *Summary:* Works in conjunction with the other Cardiac Rehabilitation team members to provide an individualized program tailored to each patient's particular needs. Assists in the assessment, planning, implementation and evaluation of patient care in all aspects of the out-patient Cardiac Rehabilitation/Prevention program as part of a multidisciplinary team. *Responsibilities:* 1. Completes initial assessment plans according to the sections policy and procedures. Reviews patients present medical status, assesses coronary risks factors, leisure time, work related activities and socioeconomic factors. 2. Plans tailored exercise program and individual goals and within the physical limitation imposed by the disease. 3. Delivers plan of care based on physician's orders, the patient's care plan and the section's policy and procedures. 4. Evaluates and documents out-patient response to treatment, exercise instruction, increase workload (RPE & THR) and counseling according to the section's policy and procedure. 5. Assists in the evaluation of the multidisciplinary teams adherence by routine monitoring of quality improvement indicators and revises indicator accordingly. 6. Serves as a hospital and community resource person in the area of Cardiac rehabilitation. 8. Performs other duties as assigned. 9. Fulfills all compliance responsibilities related to the position. *Other Information:* *Required: *Bachelor's degree in Exercise Science from an accredited program in Exercise Science or Bachelor's degree with a Master's degree in Exercise Science. Current American Heart Association certification in Basic Life Support required. Working knowledge ECG'a and arrhythmia recognition. Excellent communication skills. Salary Range: $25.70 - $47.72 hourly (Pay per years of relative experience) EOE, including disability/vets.
