Evaluation Professional Nurse (LTSR)

The Evaluation Professional Nurse is a Registered Nurse and will support Western Psychiatric Hospitals Long Term Structured Residence (LTSR) facility. LTSR is a sixteen-bed ambulatory residential program that treats various populations aged 18-65.

The Evaluation Professional Nurse is responsible for setting standards for the level and quality of care. The Evaluation Professional Nurse has responsibility, authority and accountability for the provision of nursing care. The Evaluation Professional Nurse manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication and collaboration with all team members. The role of Evaluation Professional Nurse encompasses leadership, partnership, collaboration and supervision. The Evaluation Professional Nurse establishes and maintains collaborative relationships with physicians, other health care providers, patients and their families, to achieve desired patient outcomes throughout the continuum of care. The Evaluation Professional Nurse demonstrates a commitment to the community and to the nursing profession.

Responsibilities:

·       Demonstrates accountability for ongoing professional development to enhance practice and patient care. Supports the growth of students and new staff; may serve as a preceptor. Participate in unit-specific quality improvement initiatives.

·       Engages fully as a collaborative care team member, offering assistance and partnership. Communicates effectively with the care team and advocates for patient care improvements. Develops daily patient goals and care plans that are holistic, and patient centered. Uses critical thinking to identify clinical, social, safety, psychological, and spiritual needs.

·       Contributes to efforts that improve patient care and the professional environment. Apply the nursing process using Relationship-Based Care to foster a healing environment. Uses research and evidence-based practice to enhance clinical care.

·       Maintains comprehensive documentation to support caregiver communication. Provides thorough teaching to guide patients and families through care transitions. Takes ownership of improving patient satisfaction and service quality. Reports safety hazards and promotes a safe work environment.
