Engagement Specialist - Health Homes
**Employment Type:** Full time **Shift:** Day Shift **Description:** **Engagement Specialist** - **Health Home** Troy NY - Full Time Days We are looking for an energetic individual to join our Health Home Care Management Team! The ideal candidate will be someone who exercises compassion and dedication to serving the vulnerable members of our community. Our philosophy is that recovery happens through therapeutic relationships. The sooner we can connect individuals who need ongoing Care Coordination, the sooner the member can begin to reach his or her potential. This includes the promotion of preventative care to reduce preventable emergency department and inpatient utilization, as well as an opportunity to address any social determinants of health. **Position Summary:** Responsible for engagement activities to support the development of a professional and trusting relationship with patients, care team members and healthcare colleagues in effort to continually improve the quality and continuity of care; as well as reduce inpatient hospital readmission and emergency room utilization. Accomplished by engaging and potentially enroll clients on-site at local health care facilities. Complete intake assessment, as well enrollment paperwork, and collaborating with healthcare colleagues. Support health home management and staff in development of health home marketing and outreach materials and strategies. Develop and demonstrate knowledge of the NYS Health Home goals and criteria along with care management model. **Education & Experience** Requirements **:** + Associates degree in Human Services with minimum of two years’ experience. Previous discharge planning, counseling, case management, and substance abuse treatment experience is desired. + Strong writing and communication skills are required, as well as knowledge of working with community agencies and managed care representatives. + Experience working with a diverse population and a strong understanding of multicultural issues is preferred. + A valid and insurable NYS Driver’s License. **Responsibilities:** + Incorporate best practices regarding confidentiality into all job duties and communications in accordance with policies and procedures and other applicable regulations. + Conduct outreach and engagement activities to members. + Network with the target population, community service providers, and medical providers to increase the visibility and accessibility of the health home. + Complete intake assessment to demonstrate eligibility for Health Home services, as well as enrollment paperwork. + Facilitate hand off to assigned care coordinator for longitudinal support. + Navigate electronic medical records and the RHIO + Work with the leadership to develop, assess, and implement strategies to engage hard-to-reach populations. + Complete all documentation within required timeframes (as defined in CHC Policies). It is the expectation that all interactions with or on behalf of a Health Home Member be documented in the electronic health record and be unique and detailed. Pay Range: $18.25 - $26.33 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
