EMR Chart Correction Specialist - HIM Operations

Summary: Maintains the integrity for the enterprise electronic medical record by performing or facilitating the correction of incorrect clinical and non-clinical information throughout all inpatient and outpatient settings. Responsible for ensuring all necessary corrections are made in a timely manner, validating each case prior to closure to ensure that documentation is complete and accurate to support patient safety and continuity of care. **This role has 4 openings, with one position available per shift:** + **Sunday to Thursday** + Shift 1: 6:00 AM – 2:30 PM + Shift 2: 2:00 PM – 10:30 PM + **Tuesday to Saturday** + Shift 3: 6:00 AM – 2:30 PM + Shift 4: 2:00 PM – 10:30 PM Responsibilities: • Primarily responsible for resolving certain interface errors to ensure documentation is available for patient care in a timely manner. • Develops and maintains communication with internal and external professional relationships. Communicates with end users, clinicians, ancillary health providers, and/or other operational teammates. • Performs concurrent contact moves and/or other time sensitive identity issues on second and third shift(s). Research by using a variety of systems to make final determination to process. • Partners with clinicians, ancillary & clinical departments through the necessary actions that must be taken to correct errors. • Advises multiple collaborators (clinicians, support staff, IS) as to the appropriate actions to take to quickly resolve data quality issues and minimize the impact to patient safety using a rapid response methodology. • Analyzes each reported error in Epic and assigns cases as high, medium, or low complexity based on impact to patient safety and privacy. • Creates a case, documents outreach information as well as information discovered when doing research, assigns task as needed to complete the chart correction. • Utilizes tools within Epic to complete a chart correction case such as contact move, note mover, and note redaction. Works with clinician to re-sign notes corrected. • Validates each chart correction case prior to resolution to ensure erroneous data has been corrected, corrections have been made accurately and completely to support patient safety and continuity of care. • Communicates frequently with end users throughout the chart correction process, using e-mail, telephone, in-basket messaging and Service Now updates.
