Education & Vocational Coord - CTTP Job Details Job Location Schenectady-Park Place - Schenectady, NY Position Type Part Time-Benefited Salary Range $25.00 - $26.00 Hourly Travel Percentage Up to 25% Description As an Education and Vocational Coordinator for the Clinical Time Transition Program (CTTP), you will interpret educational records and make assessments to determine the most appropriate education and vocational programs for youth in the CTTP program. You will create individualized plans for each youth encompassing the duration of their time in the program and post discharge. You will coordinate these services through relationships built within the community. You will report to the Director of the Hospital to Home (H2H) Program, and you will work onsite and in the community. Responsibilities: + Conduct needs assessments and interpret results to determine what education and vocational programs would be best suited for each youth, paying careful attention to the connection between each program type to foster success post high school + Interpret existing education records as part of the assessment process + Build a individualized plan for each youth that addresses their time in the CTTP program and their transition back to their home community + Facilitate appropriate referrals and placements to identified services + Help communication between community resources (i.e. school districts and related programs) + Train youth on job readiness skills and preparation + Work with community resource partners and employers to build relationships and job opportunities for youth Requirements and Education: + Bachelor's degree required. + 1+ years experience with school districts or other education/ vocational settings required. + Valid NYS driver's license required Benefits + Health care package: medical, dental, and vision + Tuition reimbursement + Paid time off + Holiday Pay + 401 (K) plan + Life Insurance + Loan repayment assistance + Free training/CEU opportunities About Northern Rivers At Northern Rivers Family of Services, we work together to make a difference in our client's lives.
