Its goal is to redirect frail elders from a path of hospital admission towards discharge home when it can be safely facilitated with community and medical supports.
The EDiCare Physician role presents a unique opportunity for physicians with a passion for our growing frail elderly population. The EDiCare Physician works as a member of a decade strong interdisciplinary team within the Emergency Dept at VGH with a focus on disposition planning. Frail Elders in the Emergency Department are identified and reviewed for appropriateness for a safe discharge home rather than a prolonged hospitalization, often associated with detrimental effects on a frail elder’s quality of life.
