The RN interacts with an interdisciplinary team to promote the optimal health potential of the patient and family, delivering proficient nursing care based on a detailed assessment of the patient/family needs and the hospital mission. The RN promotes a patient and family experience-oriented environment and acts as a change agent to meet the rapidly changing census and needs of all customers-both internal and external.
Dedication to acquire Bachelors of Science in Nursing within 5 years of hire. Graduation from an accredited program of nursing. Registered Nursing license in State of Ohio.”
Department Specific Job Details:
Reviews abnormal laboratory tests that resulted after the patient was discharged from an emergency services location. The ED Outreach Nurse does not review abnormal laboratory tests that result after that patient is admitted to Dayton Children’s Hospital (DCH). Reviews Dayton Children’s Radiologist final medical imaging reports, when imaging not previously reviewed Radiologist for patients seen in the emergency departments or urgent cares and discharged from the previous 24 hours to identify any discrepancies in results.
Assists families/patients with arranging follow-up appointments with specialists • Assists families/patients with arranging follow-up testing.
Answers incoming calls from patients/families with concerns about their child’s condition or care who were seen in one of the emergency services locations within the last 72 hours (about 3 days), and triages call using decision support tool or review with a provider, then documents this interaction in the patient’s electronic medical record.
Makes follow-up calls to patients/families who were seen at any emergency services location the previous day and documents calls in the patient’s Electronic Medical Record (EMR). Calls families to discuss the additional care needed based on the plan of care discussed with an emergency services provider when appropriate.
Assists with the prior authorizations for necessary prescriptions written by an emergency services provider.
