Job Title: Discharge Support Specialist – Care Management
Location: Carroll Hospital, Westminster, MD
Schedule: Part-Time | 8:00 AM – 4:30 PM
Overview:
Carroll Hospital, a proud member of LifeBridge Health, is seeking a detail-oriented and organized Discharge Support Specialist to join our Care Management team. This part-time role is essential in ensuring safe and efficient transitions of care by coordinating discharge documentation, transportation, and communication between patients, healthcare teams, and post-acute care providers.
Key Responsibilities:
Assist Care Managers with referral processes by transmitting required documentation via web-based systems and verifying receipt.
Communicate authorization status, acceptance/decline of referrals, and any follow-up requests to the Care Manager.
Track and update Level of Care (LOC) statuses; escalate issues or delays as needed.
Coordinate transportation arrangements and ensure timely communication with care teams and facilities.
Complete and document necessary discharge forms in the medical record system.
Assist in regulatory compliance by identifying patients requiring notifications (e.g., IMM, MOON, Observation Status notices).
Deliver required patient letters, obtain signatures, and ensure proper documentation in the EMR.
Provide general administrative support (e.g., faxing, copying, answering phones) to promote efficient operations.
Schedule follow-up appointments and tests as directed by Care Managers to support smooth transitions of care.
Qualifications:
Education: Bachelor’s degree preferred.
Experience: Prior experience in a healthcare setting required
Strong organizational, communication, and multitasking skills are essential.
Familiarity with EMR systems and healthcare documentation processes is a plus.
