**Employment Type:** Full time **Shift:** Day Shift **Description:** Discharge Planner/C2 - Samaritan Hospital - FT Days Position Highlights: Quality of Life: Where career opportunities and quality of life converge Advancement: Strong orientation program, generous tuition allowance and career development Work/Life: Positions and shifts to accommodate all schedules Position Summary: + Provides discharge planning as a member of an interdisciplinary team. + Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties. + Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care. Education Requirements: Bachelor's degree in related field required, or associate degree in related field with QHP required. Experience Requirements: + Experience in a healthcare/medical setting, preferred. + Strong writing and communication skills are required. + Experience working with a diverse population and a strong understanding of multicultural issues is preferred. Pay Range: $22.70 - $32.30 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
