Position Summary:

The Discharge Assistant supports the Care Coordination, Social Work staff and/or Oncology Navigators to aide in safe and efficient transitions of care. They are responsible for coordination of information and communication and other designated responsibilities for the care management team. 

Education:  

High School graduate or equivalent.

Training and Experience:  

A minimum of one-year experience in health care setting preferred.
