Disability Services Navigator **MHDS of the ECR Disability Access Point/Eastern Iowa Disability Alliance** 1 Positions ID: 77254 Posted On 08/08/2025 **Job Overview** **Disability Services Navigator** Disability Access Point (DAP) **GENERAL DEFINITION OF WORK:** This position supports individuals with disabilities in accessing services and making informed choices about long-term supports. The Disability Services Navigator utilizes the Science of Hope and person-centered planning to deliver services such as Information and Assistance, Options Counseling, HCBS System Navigation, and Service Coordination of Long-Term Services and Supports (LTSS). The role ensures that individuals are connected to appropriate resources and empowered in planning their own paths forward. Work includes in-person, online, and telephonic system navigation. **ESSENTIAL FUNCTIONS:** + Conduct intakes and complete eligibility documentation for services, including assessments and social histories. + Provide current information and assistance on available community opportunities and services. + Draft funding requests and monitor service authorizations and needs. + Determine initial and ongoing eligibility, maintaining records and making referrals as needed. + Support clients in accessing identified services to maximize self-determination. + Assist in applying for local, state, or federal benefits for clients and families. + Coordinate and monitor Individual Service Plans, including facilitating transitions and service terminations. + Advocate for clients and facilitate Interdisciplinary Team planning. + Deliver person-centered Options Counseling, including interviews, eligibility screenings, and support planning. + Participate in HCBS System Navigation and Medicaid Administrative Claiming activities. + Establish and maintain collaborative relationships with service providers, law enforcement, judicial systems, and other partners. + Attend meetings, trainings, and conferences relevant to disability services and navigation. + Perform other related duties as assigned. **KNOWLEDGE, SKILLS AND ABILITIES:** Knowledge of + Federal and state assistance programs that support individuals with disabilities. + System navigation and eligibility requirements for LTSS and HCBS programs. Ability to + Demonstrate flexibility and adaptability in a fast-paced environment. + Communicate clearly and effectively, both verbally and in writing. + Solve problems and think critically in support of client needs. + Work independently and as part of a team. + Follow oral and written instructions and resolve conflicts. + Use relevant technology and maintain computer literacy. **EDUCATION AND EXPERIENCE:** Bachelor’s degree with at least 30 semester hours in a human services field (e.g., psychology, social work, counseling, education, nursing) and year of experience delivering services to individuals with disabilities. Must possess a valid driver’s license, reliable transportation, and adequate liability insurance. **WORK ENVIRONMENT:** This role may involve a mentally stressful environment but poses minimal risk of physical injury. Regular work hours are Monday through Friday, 8:00 a.m. to 4:30 p.m., with flexibility as needed. Travel is required.
