The Director of Clinical Programs and Value Based Care is responsible for driving clinical excellence across our skilled home health operations. This role focuses on the design, implementation, and evaluation of strategic clinical initiatives that enhance patient outcomes, reduce hospitalizations, and ensure regulatory compliance. The Director will lead efforts to develop innovative clinical programs, standardize best practices, and support long-term improvements in care delivery processes. Masters Preferred + Current State license as a registered nurse + Minimum 8 years of experience as a registered nurse performing duties as a quality and operations leader + Demonstrated success in operations and quality, performance improvement programs in a home health care or related organization + Home Health OASIS and Coding Certification preferred + Valid driver’s license and insurance + Reliable transportation to perform job tasks + Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lb You will report to the VP Clinical Quality and Value Based Care. _We value the unique skills of veterans and military spouses.
