Director of Care Management Trinsic
**Job Description:** The Integrated Care Management Director has operational responsibility for care management improvement and transformation in the Ambulatory and Home environments. The Director provides strategic leadership, direction and administration in optimizing performance of care management and coordination within existing and affiliated physician practices and the House Calls Program. Under the direction of the system AVP for Enterprise Care Management, this position is responsible to design, implement and coordinate longitudinal care management by overseeing seamless transitions of care across traditional venues of care for engaged patients/members. The Director supports the development of an integrated system and learning culture, along with implements and oversees programs and partnerships designed to ensure patients/members receive the right care, at the right time, by the right provider, in the right setting to achieve the highest level of health and wellness at the best price. As the Care Management Director of Trinsic, you will: + Lead NCQA Care Management Accreditation, Survey Process, and updated annual standard review for compliance. + Lead the system-wide development of partnerships with community organizations to support Enterprise Care Management including relationship management, communication, and new program design and implementation. + Collaborate with Castell, Home Care, Clinic Management and Operations teams to develop strategic, operational and technology solutions to improve safety, quality, patient experience, and access, while reducing the cost of care delivered. + Develop growth opportunities in new states setting up workflows for multimodal care management capabilities. Minimum Qualifications + Master’s Degree in Nursing/Social Work or Healthcare Administration. Degree must be obtained through an accredited institution. Education is verified + Hold and maintain a professional license in nursing or as an LCSW + Knowledge and oversight for compliance with government regulations, contractual requirements and NCQA accreditation + Eight years of professional progressive experience in health care delivery or healthcare management + Three years of experience leading case management, disease management or care coordination programs requiring knowledge of the current healthcare environment, healthcare regulatory matters and healthcare reimbursement + Experience in a role requiring strategic thinking and planning skills and the ability to develop proposals, pilots and projects from conception to implementation. + Experience using word processing, spreadsheet, database, internet, e-mail and scheduling applications. Experience in a role requiring effective verbal, written and interpersonal communication skills. Preferred Qualifications + Professional experience working in an integrated delivery system that includes a health plan. + Experience in directing other clinical areas, working closely with physicians, and developing clinical strategies, implementing operational efforts and measuring outcomes. + Experience evaluating technology solutions to support care management functions + Experience with developing collaborative relationships with community organizations to address healthcare needs across the continuum \#LI-EXECRC **Physical Requirements:** Interact with others requiring employee to verbally communicate as well as hear and understand spoken information.
