Job Summary
The Director, Care Management is responsible for overseeing the implementation, development, and management of the Care Management and Resource Management programs. This position ensures compliance with all regulatory requirements, establishes efficient processes for patient care and discharge planning, and provides leadership to the Care Management team. The Director works collaboratively with staff, physicians, and community resources to deliver patient-centered care and achieve optimal outcomes.

Qualifications
Bachelor's Degree in Nursing requiredMaster's Degree in Nursing, Healthcare Administration, or a related field preferred4-6 years of clinical nursing experience required3-5 years of experience in care management or utilization review required1-3 years of leadership experience in a healthcare setting requiredKnowledge, Skills and Abilities
In-depth knowledge of care management principles, discharge planning, and regulatory compliance requirements.Strong leadership and team-building skills to guide and motivate staff.Excellent communication and interpersonal skills to collaborate effectively with multidisciplinary teams and external partners.Analytical and problem-solving skills to address complex patient care and operational issues.Proficiency in electronic medical records (EMR) and care management software.Commitment to patient-centered care and continuous quality improvement.Licenses and Certifications
RN - Registered Nurse - State Licensure and/or Compact State Licensure requiredBCLS - Basic Life Support within seven (7) days of hire or transfer to position required
