**9:00am-5:00pm during the week & 7:00am-3:00pm on the weekend**
**Schedule: Off every other Friday/Wednesday, and working every other weekend**
**Your Role**
**Direct Support Professional (DSP) - Transporter** for Hillcrest Family Services Residential Programs in Dubuque, IA, your primary responsibilities include transporting clients to appointments and providing in-house case coordination and treatment planning for clients. This is a flex position which you will be responsible for providing support to the other Adult and Adolescent residential programs within Hillcrest, as and when needed. In addition, you will:
+ Teaching necessary problem solving and independent living skills.
+ Providing provision of habilitation services for clients.
+ Assuring program treatment plans and IAPâs are updated on a quarterly basis.
+ Facilitate social recreation activities within the facility and the community.
+ Other DSP responsibilities on down time or if floor coverage is needed.

**What You Need**
+ High school diploma or equivalent required (GED or NCR accepted).
+ Valid driverâs license and auto insurance; ability to meet agency driving requirements.
+ Excellent time management, organization and flexibility.
+ Ability to communicate clearly, tactfully and professionally.
+ Must be able to perform the essential functions of the position.

+ Prior experience with health care documentation.

**About the Program**
Hillcrest's **Adult Residential Services**  operates **Â** several 5 bed group homes and a 60+ bed care facility for adults who have a mental health diagnosis. Serving as a steppingstone from hospitalizations to independent living, these programs provide supervision in a safe, secure, and protected environment, with the following support:
+ 24 hour staffing and supervision
+ Individual counseling and care coordination
+ Mental health support
+ Medication management
+ Social and community integration
+ Physical and emotional health development
+ Recreational activities (structured and unstructured)
+ Daily living skills education and instruction
