The position includes completing progress notes and making appropriate entries in the logbook Shift Worked: 4pm - 12am/8am - 8pm Weekly Hours: 40 FLSA Status: Non Exempt Pay range: $19.32 - $19.32 (Direct Support Professional) POSITION DESCRIPTION: + Assist participant with needs identified in the Individualized Life Plan such as, self-care, toileting, showering, tooth brushing, grooming, dressing and ensure proper hygiene + Distribute and collect linen for laundry service in accordance with procedure. + Assist participant in attaining skills needed for daily living and socialization according to their abilities + Safely accompany participant to community resources (stores, places of worship, recreation, and health facilities, etc.) + Assist with meal preparation and serving of meals. + Responsible for cleaning of serving utensils and pots. + Complete Incident Reports as appropriate. + Complete room checks as needed. + Update logbook to foster greater and meaningful communication across shifts as needed. + Demonstrate the ability to work as part of a team, + Interact with clients to promote a friendly supportive but professional atmosphere, + All other duties as assigned by the Shift Supervisor or other supervisory staff. + Attending staff meetings and in-service training. + Participate in meetings as requested. + Other related tasks that may be assigned Job Requirements: High School Diploma or Equivalency with two years of experience preferred Verbal and written communication skills essential to perform function of position.
