Dietitian Diabetes Educator - FT - 36hr - Day
*Description* Summary: Provides outpatient comprehensive diabetes self-management and medical nutrition therapy to patients and their families on all aspects of diabetes care including meal planning, diabetes medications, exercise, blood glucose monitoring, lifestyle change, and coping skills. Acts within scope of practice. Promotes diabetes education and serves as a resource to hospital clinicians. Responsibilities: 1. Completes DSME and MNT assessments and screenings using evidence based and validated tools. 2. Provides general diabetes self-management and medical nutrition education to patients and their families/care providers, to help optimize diabetes management and close knowledge gaps. Plans and implements individual and group diabetes education teaching in accordance with ADA standards using in-person and virtual visits. 3. Plans and implements individualized and group diabetes education for patients/families. Provides nutrition consultation and proposes a plan of medical nutrition therapy and administers patient education. 4. Document patient visits in a timely manner, adhering to documentation standards, coding, and billing requirements. 5. Collaborates and communicates with interdisciplinary health care team members. 6. Provides training and education to health care team members as requested. 7. Demonstrates clinical competency through continuing education and meets accrediting body regulatory standards. 8. Participate in quality improvement activities. 9. Fulfills all compliance responsibilities related to the position. 10. Maintains and models the Organizations' Values. 11. Demonstrates regular, reliable, and predictable attendance. 11. Performs other duties as assigned. Other Information: Preferred: Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) current certification preferred. Non-CDCES / BC-ADM will be considered with requirement to obtain within 18 months of hire. Non-CDCES / BC-ADM team member must have documentation reflecting completion of 15 hours of continuing education from the Certification Board for Diabetes Care and Education (CBDCE) approved CE providers annually. Minimum Experience: 1-3 years of experience preferred in a clinical setting working with individuals with Diabetes. In-depth knowledge about current diabetes treatment management. Desired Skills: Proficiency in Microsoft Word and Excel. Bilingual preferred. Working Conditions: Manual: significant manual skills/motor coord & finger dexterity Occupational: Some occupational risk Physical Effort: Sedentary/light effort.
