Description

This role will support the UCLA Alzheimer’s and Dementia Care Program by working closely with Dementia Care Specialists (DCS) to coordinate care for patients and their caregivers. Major duties include but are not limited to:

Conducts follow-up calls Schedule appointments Document patient interactions under the supervision of the DCS.  Monitoring lower-acuity patients  Performing needs assessments, and implementing appropriate program interventions.  Gathers, analyzes, and manages large sets of clinical and survey data, while maintaining compassionate communication through phone and electronic records.   Referring patients to community-based services Supporting non-pharmacologic behavioral strategies  Facilitating access to care across the health system.  Collaborates with DCS and LCSW staff to ensure accurate patient acuity tracking and provides weekly updates on patient interventions. Salary Range: $30.36 – $ 43.49/hourly
