Responsibilities
Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
This Critical Care Pharmacist will be straight evenings, 1200-2230, in an 8-days-on then 6-days-off rotation with another evening ICU pharmacist. This position will serve alongside exceptionally talented staff and receive uninhibited support from the Department of Pharmacy, Critical Care Leadership, and Mayo Clinic.

In stride with these goals, in one year alone, our department reaches over 200 scientific publication outputs and over 80 active quality improvement projects with the Mayo Clinic Quality Academy. Over 250 pharmacy staff have attained academic rank in the Mayo Clinic College of Medicine and are being increasingly recognized on regional, national, and international stages. Our department’s pharmacy residency has been accredited by ASHP since 1973. It has grown to become the primary training site for 21 pharmacy residents including 15 various specialty residencies, 12 longitudinal local pharmacy students, and 9 pharmacy summer interns from around the country. Our staff also regularly interface with and educate medical trainees including physician residents and fellows.
Daily pharmacist clinical services consist of robust daytime bedside rounding as part of a multidisciplinary team, identification and intervention of drug-related issues, provision of recommendations on optimization of patient-specific drug therapy, admission and discharge medication reconciliation, transition of care planning, routine use of expansive collaborative practice agreements, response to medical emergencies, and direct patient care on a rotating basis. As an institution valuing education, preceptorship and mentorship of learners is expected. Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources.

Qualifications
Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.

Current pharmacist license by the Minnesota Board of Pharmacy, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy.
