Coordinator Transitional Care - McLaren Careers
**Position Summary:** Evaluates acute care patients to determine appropriate admission to the Rehab Unit per federal register guidelines. Coordinates, negotiates, procures, and facilitates the care of clients to achieve quality outcomes within appropriate length of stay and resource utilization parameters for Inpatient Rehab Unit. Participates in overall case coordination with Rehab Physician, Team, and Case Management. Participates in program evaluation and quality improvement processes. **Essential Functions and Responsibilities:** 1. Employee manages acute rehab admissions: a. Employee participates in pre/post screening problem solving. Conducts tours and greets families and other decision-makers. b. Employee assesses Rehab Center patients to identify needs, rehab potential, resources and care goals. c. Employee understands or facilitates pre-admission insurance authorization process. 2. Employee maintains a working knowledge of the requirements of payers most frequently encountered in the patient population. Employee maintains responsibility for the communication of clinical information required by the payer. Employee advises patient/family of potential financial liability as appropriate via insurance of HINN/LPL per Medicare or other third party payer guidelines, and in accordance with established Medical Center/Rehab Center policies and procedures. 3. Employee will work as a liaison with internal and external Case Managers, physicians, insurance companies and community organizations. Employee establishes a means of communicating and collaborating with physicians, the Rehab Clinical Treatment Team, the patient’s payer and Administrators. Employee shares dates with team members regarding client needs, plan and response to therapeutic intervention. 4. Employee explores and implements strategies to improve client outcomes while maintaining appropriate length of stay and resource utilization parameters. a. Employee attends and maintains active involvement in the Rehab Center’s Performance Improvement Council. 5. Employee arranges and coordinates rehab team meetings. Employee demonstrates patient advocacy in the assurance of team accountability. Employee identifies variation from the rehab plan, including intervention and/or anticipated outcomes. Employee sustains team focus and provides guidance in team decision-making. **Qualifications:** **Required:** 1. Registered Nurse licensure. 2. Demonstrated clinical competency as evidenced by two years clinical experience in rehab, case management, or other equivalent achievements. **Preferred:** 1. Bachelor’s in nursing 3. Acquired and maintained knowledge of Rehab. 4. Within 12 months, attains, and thereafter maintains CRRN credentialing.
