Job Description:The Complex Care Coordinator supports the Behavioral Health Group Treatment Program, Partial Hospitalization and Intensive Outpatient levels of care by facilitating and supporting patient discharges to community-based services and higher levels of care, increasing family engagement with patients, and ensuring a smooth transition throughout the treatment process. Responsibilities include coordinating discharge referrals, scheduling outpatient appointments, providing post-discharge follow-up, facilitating psychotherapy groups, supporting the therapeutic milieu, completing suicide risk assessments, and performing administrative duties as assigned by department leadership.

Education/Experience/Skills Requirements:

Bachelor’s degree in social work, Psychology, or a related field - Required

Experience:

1 year of experience in Behavioral Health (Inpatient, residential treatment, or outpatient treatment) - Preferred

Certifications/Licensures:

Basic Life Support (BLS) – American Heart Association - Required

CPI (Crisis Prevention Institute) Training – John Muir Health - Required within 30 days of hire; renewed biannually (every 2 years)

Skills:

Proficiency in computer use, including word processing, internet navigation, and email communication - Required

Work Shift:08.0 - 08:00 - 16:30 No Waive (United States of America)

Pay Range:

$37.30 - $51.63Hourly

Offer amounts are based on demonstrated/relevant experience and/or licensure.Pay will be adjusted to the local market if hired outside of the Bay Area.
Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word ‘exempt’ on it.
