Position Overview:The Coordinator - Clinical Transplant is responsible for coordinating a multidisciplinary approach in the evaluation, treatment, and facilitation of the transplant process, as well as providing follow-up care for individuals receiving organ transplants and those who have donated. This role emphasizes collaboration and coordination, serving as both a collegial and consultative resource for nursing leadership, primary nurses, and other members of the healthcare team.Key Responsibilities:  Coordinate the comprehensive management of transplant patients, overseeing all phases from evaluation through follow-up care.Collaborate with a multidisciplinary team to ensure effective patient care throughout the transplant process.Facilitate communication among nursing leadership, primary nurses, and other healthcare professionals to optimize patient outcomes.Provide education and support to patients and their families regarding the transplant process and aftercare.Track and monitor patient progress and outcomes, ensuring that care delivery is timely and effective.Participate in quality improvement initiatives and contribute to data collection for ongoing evaluation of transplant services.Manage projects associated with transplant services, ensuring alignment with organizational goals and regulatory requirements.Qualifications:  Requires experience that demonstrates clinical competency and leadership skills.Clinical practice experience related to transplant programs is required.Strong organizational, writing, interpersonal, and verbal communication skills are essential.Critical thinking and analytical skills to navigate complex patient care situations.Ability to work independently and manage multiple tasks effectively.An Associate of Nursing (ASN) is required; a Bachelor of Nursing (BSN) is preferred. New ASN RNs hired after January 1, 2013, must complete their BSN within five (5) years from the date of hire into this position.An active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license is required.Basic Life Support (BLS) certification is required; other advanced life support certifications may be necessary based on unit/department specialty according to patient care policies.Why Join Us:
