Coordinator, Intake Support (Based in Ohio)

Job Description


Job Summary

The Coordinator, Intake Support position’s primary function is to monitor and manage the Intake Support Center/Care Coordination Portal’s daily operational activities, ensuring that both internal and external stakeholders, including individual members, can share and obtain pertinent information.  This position ensures that the information shared on the Care Coordination Portal related to a member’s services, benefits, and pertinent information along with making referrals when necessary; is addressed in a timely manner.  

 

Knowledge/Skills/Abilities

Provide daily monitoring and management of the data on the Care Coordination Portal Attend training and consultation specific to the Care Coordination Portal use and access Ensure that internal and external key stakeholders and members have access to pertinent care coordination portal information Ensure that member inquiries and concerns are addressed timely with appropriate triage to the assigned Molina department Be available as the point of contact to external Care Coordination Entities (CCEs), Care Management Entities (CMEs), the OhioRISE Plan, Individual Members, and other key providers and stakeholders as needed.  Assist with monitoring required reporting requirements to ensure that members needs are addressed timely and appropriately  Possess strong systems and technical skills as they relate to the Care Coordination Portal daily operations 1-3 years in case management, disease management, managed care or medical or behavioral health settings. Excellent organizational and communication skills Strong customer service acumen Strong computer and software use / experience Home office with internet connectivity of high speed required Possess demonstrated attention to detail and organizational abilities Possess strong interpersonal skills and exemplify excellence in customer service

 

Job Qualifications

REQUIRED LICENSE, CERTIFICATION, ASSOCIATION:

Completion of an accredited Licensed Practical Nurse (LPN) program Currently licensed in the State of Ohio, license must be active, unrestricted and in good standing.

PHYSICAL DEMANDS:

Working environment is generally favorable and lighting and temperature are adequate. Work is generally performed in an office environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Must have the ability to sit for long periods.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential function.
