Responsibilities
Performs screening and identifies needs related to the patient's plan of care, providing referral information and education; may assist with placement into nursing home, treatment center, or other venue for appropriate care and provide episodic counseling related to identified active issues.Communicates social, emotional and patient/family stressors to interdisciplinary healthcare provider teams as relates to the patient's plan of care. Advocates for advanced care planning. Assists in monitoring/trending patient outcomes to validate the effectiveness of the care plan and services provided. May also be required to meet patients and or family members either in the community, at home, or other location. Must be able to assess the environment for safety for self and patients and escalate any concerns to the Medical Social Worker, Licensed Social Worker and/or Registered Nurse member of the care team for further action.Researches and identifies community resources, vendors, medical supply companies, healthcare agencies and other resources appropriate to the patient’s individualized plan of care.
Qualifications
Required:
Master’s degree in social work2 years relevant experience or advanced degree requiredArizona: Licensed Master Social Worker (MSW:AZ)Ability to utilize psycho-social counseling skills in a focused and limited manner to support episodic crisis intervention for patients with complex psycho-social involvementExcellent customer service and presentation skills are a mustStrong interpersonal and written communication skills are essentialDemonstrated ability to apply analytical and problem solving skills Demonstrated ability to manage multiple tasks or projects effectivelyAbility to work independently as needed with a high degree of detail orientationAbility to work collaboratively with an interdisciplinary care teamExcellent computer skills and ability to learn new systemsStrong organizational (time management) and interpersonal skillsAbility to handle multiple priorities with strong attention to detailAbility to communicate effectively using written and verbal skills. Proficient in email communications and internet usage along with basic use of Google Office.Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost)Ability to work autonomously within a matrix environment without direct supervision or supportPreferred
