Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description The Registered Nurse (RN) is responsible for providing general nursing care to our patients through collaboration with Physicians and other support staff of the Center. The Registered Nurse will promote and strive to restore a patient's health through education, advocacy, and quality delivery of care. The RN is a key member of a collaborative care team, employing a whole-person care approach to actively engage patients in managing their health. This role requires exceptional teamwork, adaptability, and strong communication skills to address the dynamic and evolving needs of patients. Reporting to the Health Center Practice Manager, the RN is integral to aligning care delivery with Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) standards, ensuring high-quality, patient-focused outcomes through coordinated efforts and innovative problem-solving.   What are we looking for? + Current RN licensure in the state of Michigan. + Minimum education requirement: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN). + At least 3-5 years of relevant experience in primary care, chronic care management, disease management, home health, or ambulatory care preferred. + Experience working for a community health center or Federally Qualified Health Center preferred. +  Strong relationship-building skills with patients, staff, and providers. + Proficient in using EHR systems and Microsoft Office Suite (Excel, Word, PowerPoint). + Excellent written and oral communication skills. + Expertise in motivational interviewing and other behavioral health strategies to promote self-management. + Knowledge of physical and behavioral health medications and screening tools. +  Ability to analyze and utilize data to drive care management decisions. + Strong organizational and project management skills. + Participation in ongoing professional development and competency-based evaluations after hire are mandatory. + A flexible and positive attitude + Ability to work in a fast-paced environment  + Ability to create an excellent patient experience + Patient focused mindset What do we offer? + Competitive Wages + Paid Time off for Part-Time Employees + Paid Floating Holiday and Holidays Why work for Honor? + Honor is a patient directed and community driven Federally Qualified Health Center. + We ensure that patients come first by providing enabling services to assist patients with specific barriers to care such as transportation, translation, food security, and other social services.
