Description:

The Contignent Hospice Medical Director at Trinity Health at Home (THAH) oversees medical direction for patient and family services, collaborates with the Executive Director and clinical team, and supports Provider Relations and CPI Committee activities. They also lead the QAPI Committee.

THAH Hospice and Palliative Care physicians report to this role.

Minimum Qualifications

Licensed physician (M.D. or D.O.)Valid driver's license and access to a vehicleAbility to assess, plan, evaluate, and analyze patient careExperience in leading patient-centered careCompetence in verbal, written, and word processing communicationTime management and autonomous functioningCommitment to THAH mission and ethics

Preferred Qualifications

Board Certification in Hospice & Palliative Medicine or willingness to obtainCredentialing at hospitals serving hospice patients or willingness to obtain

Essential Functions

Oversee and evaluate medical services qualityReview and create medical policies and proceduresMaintain a network of physicians for hospice needsProvide 24-hour on-call availabilityMentor physicians and promote hospice servicesReview medical care in home or inpatient settingsConsult with the patient's physician and hospice teamDetermine hospice admission level of careCertify and recertify patient life expectancyParticipate in Interdisciplinary Team meetingsEnsure physician coverage for routine and emergency situationsDevelop educational programs for community physiciansAttend Executive, QAPI, and Ethics Committee meetingsLead the Hospice Interdisciplinary TeamEstablish and update patient care plansAssume direct medical care or assign physiciansManage inpatient hospice care if neededParticipate in on-call medical coveragePerform face-to-face visits for symptom management and certificationsAdhere to confidentiality practices
