Humana Gold Plus Integrated is seeking a Community Transition Coordinator to assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members and families towards resources appropriate for the care and wellbeing of members. The position essential functions take lead on full case management and transition services to individuals identified by maximus and long-term care coordinators via assessments/referrals, which includes service planning, placement search, and all aspects of case coordination associated with achieving a successful transition from an unhoused living situation.

The Community Transition Coordinator provides long term care service intakes, case coordination, and transition services to eligible seniors and people with disabilities who are identified on a priority list, this list comes from maximus assessors who will identify and prioritize individuals in need of the Community Transition Coordinator’s dedicated support. This is a specialized position that will work closely with the shelter providers, CCUs, outreach, hospital/rehab social workers, DHS, HFS, MCOs, family/friend supports, long term care facilities, natural supports, and other community-based resources to help individuals access long term care service plans and housing supports.
The Community Transition Coordinator responsibilities are as follows:
Meets with prospective consumers and/or their families - provides thorough explanation of services, policies, and assistance available, performs comprehensive assessment of consumer functioning, resources, and needs, and assesses consumer’s ability to perform daily living activities, medical issues, and needs and develops an initial care plan.Be present in the community – conducts outreach to locate and meet seniors/disabled population/members where they are in the community, including, but not limited to libraries, shelters, the street, and other agreed upon locations.Ensures that all necessary forms are fully completed and provides explanation of applicant's rights and responsibilities.Locates qualified providers locally and across the State of Illinois who can meet the consumers’ needs.Enters consumer’s psychosocial assessment into state computer system and provides complete description of consumer’s functional status and care plan.Performs needed assessments with seniors who meet the HUD definition of homeless, with a priority on those identified in Seniors by Name List case conferencing.Maintains accurate records to ensure proper evaluation and documentation of services. Enters thorough, objective, and timely progress notes that capture the most pertinent information for each consumer contact.Utilizes motivational interviewing to identify housing needs with individuals.Provides advocacy and direct support services including crisis intervention and short-term counseling.Helps to resolve problems or emergencies affecting the availability or quality of services and makes appropriate community referrals to support consumer well-being.Remains current on other agencies, community resources, service organizations, and programs available in the area, and provides resource and referral support to consumers and partners.Maintains up-to-date knowledge of a broad array of agency, state, and federal rules, regulations, policies, and procedures.Acts as liaison between S&DS and community partners; establishes and maintains effective working relationships and promotes S&DS programs, as appropriate.75-90% travel in Chicago and surrounding areas but may need to travel all over state for transitions (see Additional Information section).Use your skills to make an impact
Required Qualifications
The Community Transition Coordinator (Care Coordinator) must meet one of the following requirements:
Bachelor’s degree in social sciences, social work, human services, or a related field.Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one (1) year experience in conducting comprehensive assessments and provision of formal service for the elderly.Licensed Professional Counselor (LPC).Licensed Social Worker (LSW).The Community Transition Coordinator (Care Coordinator) must meet ALL the following requirements:
Must reside in Cook County in the State of Illinois (preferably in or near Chicago Midway zip codes 60638, 60459, 60501, 60455, 60629, or 60534).1+ years of experience in health care and/or case management.1+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.Ability to use a variety of electronic information applications/software programs including electronic medical records.Exceptional communication and interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.This role is considered patient facing and is a part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.This role is part of Humana’s Driver safety program and therefore requires an individual to have a valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.Work at Home Requirements

Workstyle/Travel: This is a Field position that requires 75 - 90% travel in the field to visit members. Travel is considered Chicago and surrounding areas but may need to travel all over state for transitions. Work Schedule: Monday – Friday, 8am - 5pm CST but open to a flexible work schedule.
