Assisting Hands Home Care Merrillville is looking for someone passionate about helping seniors and their families—while also building strong community relationships. If you're a people-person with a background in social work, outreach, or healthcare marketing, we’d love to meet you.
Responsibilities
Build and maintain strong relationships with hospitals, rehab centers, clinics, discharge planners, senior centers, and other referral sources
Conduct educational presentations for professionals and families on topics like dementia care, Parkinson’s care, fall prevention, and aging in place
Represent Assisting Hands at health fairs, networking events, and conferences
Track outreach activities and leads using our CRM software
Schedule and complete weekly visits with referral sources
Coordinate with internal staff to ensure quality of service and client satisfaction
Participate in strategic growth meetings and occasional on-call rotations
Share community feedback and referral trends with leadership team
Qualifications
Required: Passion for senior care and community engagement
Preferred: LMSW, LBSW, or background as RN, LPN, PT, OT, or in healthcare/social services
Excellent verbal communication and presentation skills
Organized, proactive, and relationship-driven
Proficient with Microsoft Office (especially PowerPoint) or Canva
Prior experience in healthcare marketing, home care, or community education a plus
Familiarity with CRM systems (or willingness to learn)
Reliable transportation and ability to travel locally
