**Employment Type:** Full time **Shift:** Day Shift **Description:** POSITION PURPOSE The primary purpose of your job position is to be responsible for developing relationships with community referral sources, agencies, healthcare/human services professionals, and families to provide greater understanding of the PACE Organization (PO). Community Liaison is responsible for obtaining referrals for people qualified and interested in the PO program. The Community Liaison is responsible to enter all qualified referrals into the Customer Relationship Management Platform (CRM) utilized by the PO. The Liaison will also work closely with PO staff to promote greater understanding of community needs. **Community Liaison** Trinity Health PACE LIFE at New Jersey Hours- Monday through Friday 8-5 Position Purpose: Trinity Health PACE is seeking a compassionate and proactive Community Liaison to build and maintain strong relationships with healthcare professionals, community agencies, and families. The Liaison plays a vital role in promoting the PACE program and increasing qualified referrals through strategic outreach and engagement. What You Will Do: + Develop and maintain referral networks with senior housing communities, healthcare providers, faith-based organizations, and other relevant groups. + Educate the community on PACE services via presentations, in-services, and outreach. + Collaborate with internal teams to align marketing and enrollment strategies. + Track referrals and community engagements using our Customer Relationship Management (CRM) platform. + Serve as a knowledgeable and professional liaison to ensure seamless communication between referral sources, families, and the Enrollment Team. Minimum Qualifications: + Bachelor’s degree and 2+ years of sales or marketing experience preferred (or equivalent combination). + Strong written, verbal, and presentation skills. + Proficient in Microsoft Office and CRM systems. + Ability to work independently and manage multiple priorities. + Medical clearance required prior to employment (North Carolina).
