Logan Health has a meaningful opportunity for a part-time Community Healthcare Worker to support patients and expand access to vital community resources at our Primary Care Clinic in Columbia Falls.

This position is based at our Logan Health Primary Care Clinic at 1675 Talbot Road in Columbia Falls, MT. It offers a flexible 20-hour-per-week schedule and may occasionally include weekend hours to support community events, programs, or scheduled classes. This role will also often drive from location to location during the course of business, so you must have a reliable vehicle.

What you'll be doing:

As a Community Healthcare Worker, you’ll be the bridge between our clinic patients and local community resources or programs. Whether you’re leading a class for seniors, coordinating the Food RX program, or helping a patient access much-needed resources, you’ll play a key role in improving health outcomes.

You’ll work closely with our clinic care coordinator and community partners to:

Manage and coordinate the Food RX Program, including coordinating with our local producer to distribute fresh produce and recipes to patients and community members

Host and teach community classes on topics like using the online patient health portal or accessing local resources

Develop and lead outreach events, partnering with local agencies to expand access to care and education

Connect patients with social support services such as transportation, food, housing, and financial assistance resources

Seek out local community resources and spearhead new program creation

Participate in community-focused tasks as assigned

Who You Are:

A natural connector with strong ties to the Columbia Falls community and a deep understanding of local resources

Outgoing, self-motivated, and passionate about helping others thrive

Experienced in program coordination, outreach, or community education (preferred)

Skilled in organizing events, teaching classes, and communicating clearly with diverse groups

Comfortable working independently while also collaborating with a care team

Grant writing experience is a plus!

Qualifications:

2+ years of experience in healthcare, primary care, medical office, or social services preferred

Strong knowledge of programs like Medicare, Medicaid, and VA services is a bonus

Familiarity with local social support services (housing, food access, transportation, etc.)

Current Driver’s License required. 

Driving record for the previous thirty-six (36) months must not reflect any major driving offenses (i.e; DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).

In addition, personal vehicles used for LH business:

State law minimum liability coverage limits required. 

Computer proficiency, including Word, Excel, Outlook, and data entry

Clear understanding of confidentiality laws (HIPAA)

Exceptional communication, organization, and interpersonal skills

Shift:

Variable (United States of America)

Schedule: Part-time, 20 hours per week, variable shifts. May include weekend hours based on position needs or events.

Logan Health operates 24 hours per day, seven days per week.  Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
