Community Health Worker Workleader

As a Community Health Worker Workleader, you will provide leadership and training to assigned Community Health Workers while functioning as a Workleader and performing the duties of a Community Health Worker. The caregiver in this role will support and coach other Community Health Workers in patient experience, the Pathway Hub, monthly OKRs, performance, meeting metrics and Epic documentation. **A caregiver in this position works onsite Monday-Friday, 8:00am - 5:00pm with the ability to travel locally 30% of the time.** A caregiver who excels in this role will: + Culturally mediate among individuals, communities, and health and social service systems (e.g., educating individuals and communities about how to use health and social service systems, building health literacy, and cross-cultural communication). + Assist in creating a positive and supportive work environment, enforce a safe workplace, establish a culture of teamwork and communication, create a workplace that promotes the organizational values of workplace diversity, equity and inclusion and actively promote an environment respectful of living and working in a multicultural society. + Provide coaching and social support (e.g., motivate and encourage people to obtain care and other services, support self-management or disease prevention and management of health conditions). + Advocate for individuals and communities (e.g., advocate for the needs and perspectives of communities, connect to resources, and advocate for basic needs). + Build individual and community capacity (e.g., train and build individual capacity with CHW peers and among groups of CHWs). + Develop and maintain working relationships with clients and makes referrals to other community agencies and services. + Participate in evaluation and research (e.g., engage in evaluating CHW services and programs). Minimum qualifications for the ideal future caregiver include: + A high school diploma or GED. + Community Health Worker (CHW) certification through the Board of Nursing. + Valid driver's license. + Four years of current experience as a Community Health Worker or in a patient navigation field. Preferred qualifications for the ideal future caregiver include: + Medical Assistant training or certification. + Previous peer coaching experience.
