Community Health Worker – Samaritan Hospital Mental Health Outpatient Clinic

Full time

This position works in collaboration with Samaritan Hospital Mental Health Outpatient Clinic, as well as the Integration Care Coordination System.

A Community Health Worker (CHW) is a frontline public health worker, who is a trusted member of and/or has a remarkable understanding of the community served. This trusting relationship with the community enables the CHW to serve as a liaison/link/intermediary between the Samaritan Hospital Mental Health Outpatient Clinic and community members to facilitate access to services and improve the quality and cultural competence of service delivery. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal reassurance, social support and advocacy.  This position in collaboration with an Interdisciplinary Team improves access to health and human services for individuals through in-person interactions, telephone contacts, and coordination of referrals.  Supportive training provided. 

What will you need:

High School Diploma/GED required / Associates Degree preferred.Minimum one (1) year of community involvement experience preferred.Ability to earn a CHW certificate is preferred.

Pay range: $21.50-$28.11

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
